Could be a grenade launcher, I suppose.
My intuition is some sort of suppressed special-ops thing, perhaps with a side-mounted magazine not currently inserted.
(The lip around the top end of the "barrel" suggests that to me; if it was a grenade launcher there wouldn't be much need for that, but for a suppressor, it makes sense to have a screw-on cap so you can clean/replace baffles...)

(Also, I just realized the picture zoomed in, and that revealed a scope.
That'd be weird optics for a grenade launcher, so I'm doubling down on "integrally suppressed".)

Reminds me somehow of a cross between a De Lisle carbine (https://en.wikipedia.org/wiki/De_Lisle_carbine) and a Welrod pistol (https://en.wikipedia.org/wiki/Welrod). I completely agree with Sigivald: this weapon definitely has the looks of a "suppressed special-ops" gun.
