I think it was moisture inside some of the electronics.

Moisture in Pitot tube (converts airspeed from pressure) gave false airspeed. Pitot tube heaters weren't turned on when they should have.

Pilot error.
No pitot heat = bad airspeed.
Rotate while going too slow, and you get a stall a few feet AGL.
Bad things then happen.

The camera was a remote operated by a joystick. They are used to watch taxiways that can't be seen from the tower, to help avoid runway crashes like the JLM disaster.
The only reason the footage was shot was because a controller was bored and was using the cameras to watch the B-2s take off.
